The strongest evidence for high-energy ESWT in the shoulder remains concentrated around calcific rotator cuff tendinopathy. For non-calcific impingement, results are more variable. Exercise and movement retraining remain essential.
One randomized clinical trial found that combining focused ESWT with isokinetic exercise produced better short- and medium-term improvements than ESWT alone. That supports a practical conclusion for chiropractors:

Step 3: Identify the treatment target
Use clinical palpation and, when available and within your scope, diagnostic ultrasound or imaging review to identify the most symptomatic structure.
